Rational use of pharmacological vitamin D includes a spectrum of ricketic and related disorders. Contrary to it, empirical vitamin D therapy is often prescribed for clinical situations like developmental delay, delayed dentition, chronic fatigue, loss of appetite, wide anterior fontanalle, etc. with unclear clinical rationale. Irrational and inordinate therapy, tendency of patient care-givers to self-administer for purported better bone health, influence of mass media, and easy availability of supplements have contributed for frequent clinical confrontation with previously unknown entity of vitamin D intoxication. A silent course before clinical detection and prolonged after-effects cause multisystem morbidities to health. This article tries to highlight pathophysiology, clinical correlation, and management of this lesser known entity. Clinical use of various pharmacological agents like diuretics, glucocorticoids, calcitonin, and bisphosphonates for its treatment is highlighted in the article. Keywords: Bisphosphonates, calcitonin, hypercalcemia, isotonic saline hydration, nephrocalcinosis, vitamin D intoxication
